Forklift Operator Refresher & Re-Evaluation

1910.178(l)(4)(iii) requires an evaluation of each powered industrial truck operator's performance at least once every three years, and refresher training whenever an operator is observed operating unsafely, is involved in an accident or near miss, is assigned a different type of truck, or the workplace changes in a way that affects safe operation. This is the classroom half of that cycle, for an operator who has already been trained and certified. It revisits the stability triangle and why lateral tipover is the dominant fatality mode; load centre and what an attachment does to your rated capacity; the pre-operation inspection and what takes a truck out of service; travelling, grades, and blind corners; pedestrians, which is the interaction that kills most people; dock and trailer hazards; and the tipover rule, which is counterintuitive and has to be automatic before you need it. The practical evaluation cannot be done online. Your employer must observe you operating the actual truck in your actual workplace and certify it under 1910.178(l)(6) — this course is the knowledge portion that accompanies it. Issues a Migna certificate of completion.
