Confined space entry: the roles of entrant, attendant, and supervisor
More than half of the people who die in confined spaces are would-be rescuers — coworkers who saw someone go down and climbed in after them. That single statistic explains the entire structure of OSHA's permit-required confined space standard: it turns rescue from an act of instinct into a planned system with clearly divided roles. Get the roles right and one victim stays one victim.
The three roles
A compliant permit entry assigns three distinct roles, and they don't overlap:
The authorized entrant is the person who goes inside. They know the hazards and the symptoms of exposure, use the required equipment (including a retrieval harness and line where required), stay in communication with the attendant, and — critically — exit immediately on any order to evacuate, any alarm, any symptom they notice, or any prohibited condition. There's no "let me just finish this weld first."
The attendant stays outside the space, the entire time, and does nothing that would distract from monitoring. The attendant tracks exactly who is inside, watches conditions inside and outside, keeps unauthorized people away, maintains communication with the entrants, orders an evacuation the instant anything degrades, and summons rescue. The one thing an attendant does not do is enter the space — not even to save an entrant — unless they are relieved by another attendant and are trained and equipped for entry rescue under the program. An attendant who abandons the post to attempt a rescue removes the one person watching everyone else.
The entry supervisor authorizes the entry. They verify that the permit is complete and correct, that testing and isolation are done, and that rescue services are available and able to respond — then they sign off. They also terminate the entry when the work is done or conditions change.
Why the paper comes before the people
Before anyone crosses the plane of the opening, the entry permit documents the space, the hazards, the acceptable atmospheric conditions and the actual test results, the isolation measures (lockout, line blanking, ventilation), the rescue arrangements, and the names of the authorized entrants and attendants. The permit is posted during the entry and canceled when it ends.
Rescue is designed, not improvised
The preferred method is non-entry rescue: entrants wear a harness attached to a retrieval line running to a mechanical device (a tripod and winch) outside, so an unresponsive entrant can be hauled out without anyone entering. Entry rescue — a trained team going in — has to be arranged before the entry, with confirmed capability; "we'll call 911" only counts if the responding service has confirmed it can perform this specific rescue in time. Most local fire departments are not equipped for it by default.
The hardest rule in confined space work is also the most important: if a coworker collapses inside, you sound the alarm and let the planned rescue happen — you do not go in. Everything about the permit system exists to make that restraint possible.
